PIAA CIPPE Physical FORM Sections 1 - 6
- If you HAVE completed the online registration, only sections 5 - 6 must be completed for your PIAA certified physical - upload section 5 & 6 here
- If you HAVE NOT completed the online registration, the entire CIPPE sections 1 - 6 must be completed for your PIAA certified physical. Please turn the completed sections 1 - 6 into the high school athletic office. Upload sections 1 - 6 here
PIAA CIPPE - Re-Certification Form - Sections 7 - 8
- fill this out when you have already had a physical (dated June 1st or later) and are signing up to play another sport in the same school year. Complete section 7 and turn in to the high school athletic office. If you checked "yes" to any of the 6 boxes on section 7 you must secure the clearances of a physician or P.A. and have them fill out and sign section 8, then turn sections 7 and 8 in too the athletic office.
